Metabolic Care Program

Our comprehensive program is personalized to fit your health issue and goals. Based upon a personalized consultation, we design a program that can include:

Health Coaching

Health coaching is a patient-centered approach that uses one-on-one discussions to address present or future health issues. Discussions between a health coach and patient encourage self-discovery in addition to education and accountability in health behaviors. Using personal accountability and education, your health coach will help you create a wellness plan that addresses your specific health issues and goals.

Cognitive Behavioral Therapy

Intervention through behavioral therapy performed by a licensed counselor will be tailored to your individual needs. To meet health and wellness goals, behavior changes must be made. Self-monitoring, goals setting, and problem-solving skills will help you meet your goals.

Dietary Counseling

Medical nutrition therapy and nutrition counseling services are provided by our Registered Dietitian. The Registered Dietician will provide you with practical skills to apply nutrition education into daily life. The focus will be on making small changes that have a BIG impact on overall health.

Diabetes Prevention Program

The Diabetes Prevention Program (DPP) offered through the YMCA shows that people at high risk for type 2 diabetes can prevent or delay the development of the disease by losing a modest amount of weight through lifestyle changes, including changes in diet and exercise. Participating in the DPP has been shown to decrease insulin resistance and prevent progression to diabetes.

Culinary Medicine

Culinary medicine uses an evidence-based approach to translate nutrition research into practical use in home kitchens. It is an emerging field that combines the art of cooking and the science of medicine. The innovative curriculum used at GCCM employs a food first approach to nutrition and focuses on simple techniques and culinary skills to ensure that eating healthy is enjoyable and sustainable for all.
